Can we talk about people mulching right up against tree trunks?

I was on a job last week removing a dead maple in a backyard near Portland and when I cut it down, the bark was rotting right at the base. The homeowner had piled mulch about 8 inches deep against the trunk like a volcano. That trapped moisture and killed the cambium layer over a couple years. I see this all the time now. Has anyone else had to explain why mulch should look like a donut, not a mountain?

A landscape designer told me mulch volcanoes kill trees slowly but surely.
