Found a rat's nest of extension cords behind a fridge in a 1920s apartment building.
The owner said it had been 'fine for years' but the insulation was so brittle it crumbled when I touched it, so what's the worst 'it's always been like that' fix you've had to undo?

My sister's neighbor lost their whole kitchen last year. It was a toaster oven that shorted out while they were asleep. The fire marshal said it was a known issue with that model. Sometimes the safety features just fail, and it's not about luck. I'd never leave one plugged in after seeing that.

Maybe @riley_price just got unlucky with a faulty unit. Most appliances are pretty safe if you follow the instructions. It's probably fine for normal use.
