Did you try switching to a fluoride rinse after flossing, that's what finally worked for me.

My buddy Dave was the same way, brushed twice a day and flossed every night for like 6 months straight and still ended up with two cavities last visit. I used to think flossing was enough to keep everything perfect, but after seeing that I'm starting to realize it's more about what you eat and how your teeth are shaped. Some people just have deeper grooves in their molars that trap stuff no matter how much you floss. Plus if you're not using fluoride toothpaste or getting enough calcium that can mess you up too. It's frustrating but I guess it proves there's no magic fix, just a bunch of little things that add up.
