I was setting up on a tight site in Boston last week and realized I've been eyeballing my outrigger pads wrong for years.

Man, I've been there. I used to just slap the pads down on what looked flat. Then one time on a slight slope, the whole rig had a little shimmy I did not like. Now I break out the level every single time, even if the ground looks perfect. It's one of those things you don't question until you see it move.

Yeah that "shimmy you don't question" from @jenniferb53 is the real warning sign. Once you feel that, you never skip the level again.

That shimmy Jennifer mentioned is no joke. My crew was on a site in Lowell last year with maybe a 2 inch slope we ignored. The whole crane cab had a slow sway you could feel just standing next to it. What kind of level are you using now, just a standard 2 foot carpenter's level or something longer for more accuracy?
